General question about broken quests

This question is about broken quests in general, not warlock specific quests, so posting it in this forum.

Over the past week, I have encountered three broken quests. One requires you to find Hammertoe the dwarf in Uldaman. The second requires you to kill Khan Jehn the centaur in Desolace. The third requires you to find the missing night elf courier in Feralas.

For Hammertoe, I found him but there was no "?" over his head so I could not progress. For Khan Jehn, he was inside his tent, but for some reason he was stuck in place. I could not pull him outside the tent and any damage I did to him was quickly regened. He also never attacked back when I got within melee range. For the missing courier, the pod he is stuck in was not Opernable so I could not rescue him.

In guildchat, other people who had tried completing these quests stated they had similar problems. The only solution they said was to wait until there was a server reset and hope that they aren't bugged the next time I try to complete them.

It appears that for all three quests, they initially are working fine after a server reset, but over time after multiple players complete the quests, they somehow become broken and uncompleteable.

So a couple of questions.

1. For bugged quests like this, are the problems consistent across all the servers or are they generally specific to individual servers?

2. Anyone ever had any luck with a GM petition to reset the NPC before the regular scheduled server maintenance?

3. Were problems with quests like these prevalent in the closed beta?

I'm a software engineer by profession so somewhat familiar with the concept of code reuse. I would assume that a large majority of the quests use the same basic code design. In other words, all the quests that require you to kill X number of mobs use the same base code module, quests that require you to talk to a NPC use the same module, collect X number of items, and so on.

For the most part, almost every quest in the game works perfectly fine. Assuming they all use the same basic code design, I'm curious to know what causes some of them to break over time.

General question about broken quests

I have a quest that on the last leg the final NPC does not respond and has no question mark. It is especially frustrating when it is the last part of a mutlipart quest.

It is my guess that it is not so much a problem with the code, but rather a problem with the data or flags of the quest itself.
