One thing you said you went from Gloves of Malefic to Enslaved Doomguard Grips. I think you should go back to the T6(since they are better) and you can then get rid of even more hit gems. also the Belt of Blasting is better than the noose, so keep that equipped. Just drop your hit gems and you should be good to go. As for when to LT/DP, the best times to use them is when you have to move. I would say an ideal time to LT is after every dps cycle(i.e dots and sbolts). Best time to DP is when you're >75% hp. and yes you should try to keep your hp = mana.
